Services

Life Coping, Inc. helps the elderly and adult disabled individuals, along with their families, navigate the complexities of day-to-day living.

Why Case Management?

“Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual and family’s comprehensive health needs through communication and available resources to promote quality cost-effective outcomes,” according to the Case Management Society of America (CMSA). Activities related to case management include care coordination, complex condition management, “population health management” through wellness, disease and chronic care management, and promoting transitions of care services.

America’s aging population is creating a demand for services to help elderly citizens manage medically complex situations as well as basic living needs. Life Coping case management provides a trustworthy, ongoing, consistent, local liaison for families with aging parents, closing the gaps between medical services, home help, nursing care and help with the financial questions surrounding health care. We make sure that important issues don’t “fall through the cracks,” that doctors don’t prescribe conflicting medications, and that elderly clients aren’t overwhelmed by confusing administrative demands.
